question 1 no you can't upgrade pre nightfall items or armor.
question 2 if you have a lvl 20 rit that can run the solo farming build or the new zero dmg 55 monk, i know of a good spot to farm insignias , upgrades and runes out side of "Jennur's Horde" in "Vehjin Mines" it is also a very good spot to do chest runs almost always 1 chest around there
also being as your somewhat new to nightfalls i thought i'd mention a tip and this is why i pm'd you insted of posting on your forum post.
and that is that if you do happen to find item upgrade "highly salvagable" expert slavage it off the item and save it use those for putting onto ruby daggers and diamond or spphire items they have a % chance to salvage into rubys,diamonds and saphires.
