Choppy Graphics & Distortion 

For awhile I've been having this kind of issue where the game will get very choppy and then the graphics will start tearing. I have fairly high-end overclocked hardware- Athlon XP @ 2375 Mhz, 1.5 GB ram @ DDR400, Ati 9700 Pro @ 370/350

What I did to correct the problem at first was simply to save and reload the game. However, I discovered that changing the shadows graphics setting from "all complex" to "party complex" seems to have fixed the issue entirely.

A lot of "tearing" happens because of having vsync turned off. Try turning vsync on if it's off, or at least set it to application preference.

Vsync's always off. Maybe tearing is not the correct term. Spell effects start going haywire. Polygons and wire frames convolute break free from their textures. The framerate drops to about 5fps. It would happen a couple of times per day.

Your comment about the shadows makes me think your video card might be having an issue with DX9 pixel shaders, which is possible considering it's an older card. Have you noticed any more problems since turning the shadow detail down?
