About 2-4(varies). Try to use holds in different ways, such as having another type of dominator with you. This might not be true, but I find that stacked holds are better when they're different(strangler(plant conrtrol) and dominate(mind control) together makes a more effective hold then clicking strangler twice).

3 or 4 sounds about right, just make sure they are strong i guess.

I had an ice/ dom and to hold i had to use frostbite once the block of ice twice so about 3-4 is right. At first frostbite only slows the enemy.

FYI, you cannot hold an AV/Hero when he has purple triangles up no matter how many holds you throw.

There is no set way to measure this across all the AVs/Heroes. Each one has their own different level of Mez Protection and each hold has its own magnitude level and therefore it really depends on the holds you are using and then on who you are fighting. So really, there is no set number of holds it will take.
Oh, I'd also like to point out that holds aren't the only type of power that effects an opponents Mez Protection. Knockback/ups, Immobs, Sleeps, and Disorients can all effect the AV/Hero in the same way a hold does before the hold's effect goes off.
