The portal to the Realm of Torment can be found in the desolation as a part of the primary quest line.
Deldrimor war camp is a town in Grenth's footprint. It allows easy access to sorrows furnace.

Sorrows Furnace is not required to get through the story-line. In fact, it has barely any ties with the storyline of prophecies. The only story i found within the sorrow's furance was the battle between the dredge and the stone summit dwarves. But if you haven't explored any of faction yet, I will leave that for you to find out.
The realm of torment is a deadly place. I won't tell you what you will find in there but you can get there through the Mouth Of Torment (an outpost) Or from the mission before it which will take you to the outpost (the name escapes me, can someomne else post the name of it) If you haven't explored this far in nightfall then I suggest you start now, this for me was one of the greatest (yet most frustrating) part of Guildwars to play and will require you're chracter to be well equipped with max armour and wield a good standard weapon. Some of the missions within are very tedious as well, but don't let it throw you off. It is matched up by the awesome loot there.
Once you have completed to Realm of Torment, you will aquire permanent access to the Domain of Anguish (DoA) via the Chantry of secrets (elona's version of ToA)
Good luck!
