What does it mean when you slot a power?
Example:
"Level 04 : Death Shroud- a good AoE, but not to be used when teaming, as you will steal the tank's aggro and die

As far as the slotting used in the example above, the number he has in parenthesis is the level that he put a slot, or enhancement slot, in the power.
I'm guessing you're new to the game, so I'll give you a little more background. At certain levels you get to select a new power. Up until you reach level 32, you can select a power at every even level. Whenever you advance to an odd level (3,5,7) you can add enhancement slots to your powers which gives a power the possibility to become more effective.
Now, enhancements are upgrades you add to your powers to make them more effective. The maximum slots you can have in any one power is 6.

First...welcome to the site
On to your question. As your hero/villian levels you get to add 2 "slots" on the odd levels. When you "slot" a power, you are adding an enhancement that will make your power better.
In your example, a damage enhancement was added at level 4 and 17, an accuracy at 19, and endurance reductions at 19 and 21. The damage enhancement will add to the damage of that power, accuracy adds accuracy, and endurance reduction makes the power use less endurance.
